  2. Call 512-710-0551 and select the Medical Records option
  3. Send a message through the Three Oaks Counseling patient portal:  https://www.therapyportal.com/p/threeoakstexas/
  4. Select the “Medical Records?” quick link on the Three Oaks Counseling website:  https://threeoakstexas.com/

Records will be provided in accordance with Texas law and applicable privacy regulations.
